As an example, you are a baby boomer, your parent is sick, they live in another state, YDP can help. They will contact all their doctors and provide a comprehensive analysis of the care plan. They will coordinate the plan with all the primary care and specialty physicians and assure that outcomes of care are managed. (Services available in USA and internationally). 2. Primary care. If you are unsatisfied with your current HMO, doctor or care plan, YDP will provide a primary care physician and assistant for your needs.
