In order to subscribe, two different forms must be filled online. In the first form the individual must select his desired gift from a list, and then furnish his e-mail address. Once this form has been submitted, the interested party must fill in a second form, this time detailing personal particulars such as first and last name, and date of birth. In addition to that, contact information must be supplied, such as street address, city of residence, State, and county. Finally, a phone number must be provided as well. The website lists the requirements that must be complied with in order to receive a free incentive gift. Besides providing valid information, at least eight silver, eight gold, and eight platinum offers must be completed.
