This site will provide you with all the information you need to join thousands of Illinoisans who have chosen their medical home already. You will be able to find many different sections, such as a Provider Section, where you will find provider information, frequently asked questions, provider access, education, provider newsletter, clinical resources, PCP application, specialist registration, and quality tools. You will also be able to find events calendar, important links, advisory subcommittees, and many others. You can also find a Client section, with client frequently asked questions, client newsletter, you can select your own provider, or the enroll category, where you enter to join the Illinois Health Connect, first by picking a PCP, and then following the rest of the steps to make.
